Currently there are a variety of employers who pay
a varying rates and conditions. As of July 2007 some employers are now paying
.5% of the gross wages into a fund for education of people working with the
I.A.T.S.E. Local 16.
PLEASE check with the business agent assigning the steward
to confirm the terms and rates of the contract you are working under.
These forms are not guaranteed to be correct for
all contracts.

unsure as to the correct vacation rate and H and W contributions for the employer.
These forms are created by Geoffrey Linder. There are 3 basic forms, one page, two page and three page.
After downloading the forms and opening them you will notice
that there is a request for a password. The reason for the password is to
prevent overwriting the blank template and loosing a clean copy.
Just do a 'SAVE AS' to a different name and all of a sudden the document is
no longer locked.
